Full Vida Therapy offers evaluation-only mental health assessments for individuals who need professional documentation related to work, school, housing, or disability accommodations.
These services are designed to assess whether current mental-health symptoms and functional impairment support formal documentation.
This service does not include therapy or ongoing treatment
What This Service Is (and Is Not)
This is a one-time clinical evaluation for documentation purposes only
This service includes:
A clinical interview
Review of symptoms and functional impairment
Mental Status Exam (MSE)
Standardized screening tools (e.g., PHQ-9, GAD-7)
Completion of one primary form or one primary narrative letter, when clinically supported.
This service does not include:
Psychotherapy or ongoing treatment
If ongoing therapy, psychiatric care, EMDR, trauma-focused treatment, or additional support is clinically recommended following the evaluation, referrals to trusted and carefully vetted clinicians and providers can be provided upon request.
Crisis intervention or emergency services
Advocacy with employers, schools, or disability programs
Legal representation or testimony
Guarantees of approval or outcomes
Ongoing disability management or recurring disability certifications through state disability programs (such as California EDD/SDI)
Emotional Support Animal (ESA) letters
Independent Medical Examination (if applicable)
Standard documentation is completed within 5-7 business days of your evaluation.
Who This Service May Be Appropriate For
This service may be a good fit if you need:
FMLA paperwork related to mental-health symptoms
Short-term disability documentation based on psychological impairment
A workplace accommodation letter related to mental-health functioning
A school accommodation letter for academic support
Appropriate for individuals needing independent mental-health documentation when requested forms fall outside the scope of primary care or when a behavioral health assessment is clinically indicated.
Individuals requesting housing-related disability documentation, including reasonable accommodation letters or other housing documentation when clinically supported.
You do not need to be an existing therapy client to request an evaluation.
How the Process Works
1. Request Your Evaluation
Complete the inquiry form on this page and select the type of documentation you are requesting.
2. Complete Your Intake Paperwork
You'll receive a secure client portal invitation by email where you'll complete the required consent forms, intake paperwork, and brief screening questionnaires. If you have a specific form, job description, or supporting records, you may upload them through the portal for review.
Please Note: Your secure client portal invitation will remain active for 5 days. If the required paperwork is not completed within that timeframe, your portal invitation will be closed and you'll need to submit a new evaluation request if you wish to proceed. No appointment is scheduled until your intake paperwork has been completed.
3. Schedule Your Evaluation
Once your intake paperwork has been completed, you'll be contacted to schedule your one-time telehealth evaluation.
Please Note: Payment is collected on the day of your scheduled evaluation, prior to the start of your appointment.
4. Complete Your Evaluation
We'll meet for a comprehensive one-time psychological evaluation to assess your current symptoms, functional impairment, and determine whether the requested documentation is clinically supported.
5. Clinical Review & Documentation
Following the evaluation, your clinical interview, screening results, and any supporting documentation are carefully reviewed. If the requested documentation is clinically supported, it will be prepared according to the turnaround time you selected when requesting your evaluation.
6. Access Your Documentation
Your completed documentation will be securely uploaded to your client portal, and you'll receive an email notification when it's ready. You may then download, print, or submit the documentation directly to your employer, school, housing provider, insurer, or other requesting party.
Please Note: Documentation is provided directly to the client. If a third party requires additional certifications or documentation outside the scope of this independent evaluation service (such as California EDD/SDI online provider certifications or ongoing disability updates), those may need to be completed by a treating provider.
Clients must be physically located in California at the time of service.
About Your Evaluator
Viviana McGovern, LMFT #136563 has completed 100+ workplace and disability evaluations and specializes in anxiety, PTSD, burnout, ADHD, and neurodivergence — the conditions most commonly underlying accommodation requests. Evaluations are conducted via secure Telehealth and documentation is written to meet employer, school, and disability program standards.
Information for Employers & HR Departments
Full Vida Therapy provides neutral, evaluation-only mental health assessments for documentation purposes.
Documentation is completed only when clinically supported based on functional impairment at the time of evaluation.
The clinician does not serve as an advocate for employees or employers and does not guarantee outcomes.
Communication with employers or HR departments occurs only with client authorization and is limited to clarifying or confirming information already documented.
This service is designed to support clear, ethical, and efficient documentation while minimizing administrative burden for HR teams.
When employers request clarification or challenge submitted documentation, supplemental letters are available at an additional fee. A signed Release of Information is required.

Each evaluation includes one primary form or one narrative letter, when clinically supported.
Requests for additional forms, employer or insurer questionnaires, clarification requests, challenge responses, addendums, revisions, or extensions are separate services and may require an additional fee.

All services are self-pay. Payment is required prior to the evaluation.

The clinician does not engage in ongoing back-and-forth communication with HR departments, schools, or disability reviewers.

No. You do not need to be an existing client at Full Vida Therapy. This service is open to anyone who needs mental health documentation and is located in California.

The evaluation is a one-time telehealth appointment that typically lasts 45 to 60 minutes. You will also complete intake paperwork and brief screening tools before the session.

Full Vida Therapy provides independent psychological evaluations and any clinically supported documentation resulting from those evaluations. The practice does not complete provider certifications through the California EDD/SDI online portal.
If you are applying for California State Disability Insurance (SDI), you may need a treating provider to complete the EDD certification and any required ongoing disability updates. You are welcome to share your evaluation documentation with your treating provider if clinically appropriate.

You will need to provide the exact form required by your employer, school, or disability program before the appointment. Generic letters are not accepted by most employers — having the specific form ensures your documentation meets their requirements.
If no specific form is required, please let us know before your appointment so we can determine whether a narrative letter is appropriate.

Standard turnaround is 5 to 7 business days following your evaluation. Rush processing is available if you need documentation sooner — 24 to 48 hours (+$300) or 48 to 72 hours (+$200).
Your evaluation includes one primary form or one narrative letter, when clinically supported. Additional forms or documentation requested after the initial evaluation are billed separately.

If your employer requests clarification, additional information, or challenges the documentation submitted, a supplemental response letter can be prepared for an additional fee of $150–$200. A signed Release of Information is required before any communication with your employer or HR department takes place.
Requests for additional forms, insurer questionnaires, disability reviewer requests, or supplemental documentation beyond the original evaluation may require an additional fee depending on the scope of the request.

Direct communication with third parties requires a signed Release of Information from you. Any communication is limited strictly to confirming information already documented in your evaluation — nothing beyond that. The clinician does not advocate on your behalf or engage in ongoing back-and-forth with employers or reviewers.

No. Documentation is completed only when clinically supported based on your symptoms and functional impairment at the time of evaluation. The fee covers the clinical evaluation itself and is non-refundable regardless of the outcome or any decisions made by your employer, school, or disability program.

Yes. School accommodation letters are available for students who need academic support documentation based on mental health symptoms. The process is the same — a one-time evaluation via telehealth followed by a letter when clinically supported.

Extensions, renewals, and updated forms each require a new evaluation and updated screening tools, as documentation reflects your clinical presentation at the time of service. Renewal evaluations are $250.

There is no fixed list. Evaluations assess current symptoms and functional impairment at the time of the appointment. Conditions that commonly support workplace or disability documentation include anxiety disorders, depression, PTSD, ADHD, burnout, adjustment disorders, and other mental health conditions that affect day-to-day functioning.

No. These evaluations are independent clinical assessments. Documentation is completed only when supported by the evaluation findings. Recommendations are based on clinical judgment rather than a requested outcome.

No. Recommendations are based solely on the clinical findings from your evaluation. While your requested documentation helps me understand your goals, any recommendations provided are based on my independent clinical judgment and the level of functional impairment identified during the evaluation.

Prior records and evaluations are welcome and are reviewed as part of the evaluation process. However, I do not offer complimentary record reviews or determine whether existing documentation is sufficient outside of a scheduled evaluation.

No. Full Vida Therapy does not provide Emotional Support Animal (ESA) letters as part of this evaluation service. This service is limited to independent psychological evaluations for workplace, leave, disability, school, and housing-related accommodation documentation when clinically appropriate.
Full Vida Therapy provides evaluation-only documentation services in accordance with ethical and legal standards for mental-health practice in California.

Employment decisions, legal obligations, and compliance with applicable laws (including ADA, FEHA, and FMLA) remain the responsibility of the employer and employee through the interactive process. Full Vida Therapy does not provide legal advice, make employment decisions, or advocate for specific legal outcomes. Recommendations reflect the client's presentation at the time of the evaluation and do not establish an ongoing treatment relationship or ongoing disability management.
All evaluations require signed informed consent prior to scheduling. Reports are based on information obtained through clinical interview and assessment and are released only with appropriate authorization. In some circumstances, records or reports may be subject to subpoena or legal review.
